Implementasi Layanan Home Care Dalam Meningkatkan Kualitas Hidup Pasien Di Masyarakat

ABSTRACT
Home care services represent an innovative approach in healthcare delivery aimed at improving patients' quality of life, particularly for individuals with chronic illnesses, elderly patients, and those requiring post-hospital care. This community service program (PKM) aims to implement home care services to enhance the quality of life of patients within the community. The methods used include health education, home visits, patient condition monitoring, and family empowerment in providing self-care. The target group consists of communities with limited access to healthcare services in the assisted areas. The results indicate an improvement in family knowledge regarding patient care, increased treatment adherence, and better health conditions and quality of life among patients. The implementation of home care services has proven to be an effective holistic approach to improving patient well-being in the community.

ABSTRAK
Pelayanan kesehatan berbasis rumah (home care) merupakan salah satu inovasi dalam sistem pelayanan kesehatan yang bertujuan untuk meningkatkan kualitas hidup pasien, khususnya pasien dengan penyakit kronis, lansia, serta pasien pasca perawatan di fasilitas kesehatan. Kegiatan Pengabdian kepada Masyarakat (PKM) ini bertujuan untuk mengimplementasikan layanan home care sebagai upaya meningkatkan kualitas hidup pasien di masyarakat. Metode yang digunakan meliputi edukasi kesehatan, kunjungan rumah, pemantauan kondisi pasien, serta pemberdayaan keluarga dalam perawatan mandiri. Sasaran kegiatan adalah masyarakat dengan keterbatasan akses layanan kesehatan di wilayah binaan. Hasil kegiatan menunjukkan adanya peningkatan pemahaman keluarga terhadap perawatan pasien, peningkatan kepatuhan pengobatan, serta perbaikan kondisi kesehatan dan kualitas hidup pasien. Implementasi layanan home care terbukti efektif sebagai pendekatan holistik dalam meningkatkan kesejahteraan pasien di masyarakat.
