Program Home Care Untuk Meningkatkan Kualitas Hidup Lansia Dengan Penyakit Degeneratif

ABSTRACT
The increasing elderly population has led to a rise in the prevalence of degenerative diseases such as hypertension, diabetes mellitus, and osteoarthritis. These conditions often reduce the quality of life of the elderly due to physical limitations and the need for ongoing healthcare. Home care programs are a healthcare approach implemented in the patient's home to provide ongoing care and education. This Community Service (PKM) program aims to improve the quality of life of elderly people with degenerative diseases through a home care program. The program's implementation methods include identifying the elderly's health conditions, providing health education, monitoring their health, and assisting families in caring for them. The program was conducted with 30 elderly people with degenerative diseases over a two-month period. Results showed an increase in the elderly's and their families' understanding of degenerative disease management, as well as increased adherence to medication and a healthy lifestyle. The home care program has been proven to have a positive impact on improving the quality of life of the elderly and strengthening the family's role in caring for the elderly at home.

ABSTRAK
Peningkatan jumlah populasi lansia menyebabkan meningkatnya prevalensi penyakit degeneratif seperti hipertensi, diabetes melitus, dan osteoarthritis. Kondisi ini sering berdampak pada menurunnya kualitas hidup lansia akibat keterbatasan fisik dan kebutuhan perawatan kesehatan yang berkelanjutan. Program home care merupakan salah satu pendekatan pelayanan kesehatan yang dilakukan di rumah pasien untuk memberikan perawatan dan edukasi secara berkelanjutan. Kegiatan Pengabdian kepada
Masyarakat (PKM) ini bertujuan untuk meningkatkan kualitas hidup lansia dengan penyakit degeneratif melalui program home care. Metode pelaksanaan kegiatan meliputi identifikasi kondisi kesehatan lansia, edukasi kesehatan, pemantauan kondisi kesehatan, serta pendampingan keluarga dalam perawatan lansia. Kegiatan dilakukan kepada 30 lansia dengan penyakit degeneratif selama dua bulan. Hasil kegiatan menunjukkan adanya peningkatan pemahaman lansia dan keluarga mengenai pengelolaan penyakit degeneratif serta peningkatan kepatuhan terhadap pengobatan dan pola hidup sehat. Program home care terbukti memberikan dampak positif dalam meningkatkan kualitas hidup lansia serta memperkuat peran keluarga dalam perawatan lansia di rumah.
